Insurance claim rejection can turn a medical emergency, accident or family loss into a serious financial crisis. In most cases, the issue is not the event itself, but mistakes made before or during the claim process.

For Indian policyholders, the message is clear. Your insurer decides a claim based on the written policy contract, disclosures made at proposal stage, and documents submitted as evidence. Verbal assurances from agents rarely help if the policy wording says otherwise.

Insurance claim rejection in India: Why valid claims fail

Many families buy health, life, motor or travel insurance and assume the policy will automatically pay when trouble comes. That is a risky assumption. Insurers check whether the event is covered, whether the policy was active, whether waiting periods are over, and whether the claim papers support the loss.

The Insurance Regulatory and Development Authority of India, or IRDAI, has strengthened policyholder protection through regulations on disclosure, servicing and claims handling. IRDAI has also stated through its policyholder resources that genuine claims should not be rejected only because of technical delay in intimation where the delay was unavoidable.

But this does not mean every rejected claim will be paid on appeal. If there is material non-disclosure, fraud, an inactive policy, or a clear exclusion, the insurer may still repudiate the claim. Repudiation means formal rejection of a claim.

Claim rejection reasons policyholders must watch

The most common insurance claim rejection reasons are avoidable. They usually begin at purchase stage, not claim stage.

1. Non-disclosure of medical history or material facts

This is one of the biggest reasons for health and life claim disputes. Material facts are details that affect the insurer’s decision to issue a policy or price the risk. These include diabetes, hypertension, surgeries, smoking, alcohol habits, risky occupation, or existing symptoms.

For example, if a policyholder hides long-standing diabetes and later files a cardiac claim, the insurer may argue that the risk was not disclosed correctly. Always fill the proposal form yourself. Do not let an agent tick boxes without your review.

2. Exclusions, waiting periods and sub-limits

A health policy may exclude certain treatments for a fixed period. Maternity, joint replacement, cataract, pre-existing diseases and specific illnesses often carry waiting periods. A waiting period is the time during which a listed condition is not covered.

Sub-limits also matter. If your health policy has a room rent cap and you choose a higher category room, the insurer may apply proportionate deduction on the total hospital bill. This can sharply reduce the claim payout.

3. Lapsed policies and unpaid premiums

If the premium is not paid and the policy lapses, the risk cover may stop. In life insurance, a lapsed policy can leave nominees without the expected payout. In health and motor insurance, a break in renewal can also affect continuity benefits and claim eligibility.

Set auto-debit instructions, track renewal dates, and keep premium receipts. For business owners, CAs should ensure key policies are included in compliance and renewal calendars.

Insurance claims documentation: The checklist that matters

Poor documentation can delay or damage even genuine insurance claims. Insurers and TPAs, or third-party administrators, rely on records to verify the event, cause of loss and amount payable.

Keep a digital and physical claim folder with:

  • Policy copy, premium receipts and insurer contact details
  • Proposal form and medical declaration copy
  • Hospital bills, prescriptions, test reports and discharge summary
  • FIR, photos, videos and repair estimates for motor or theft claims
  • Death certificate, nominee KYC and bank details for life claims
  • Travel tickets, cancellation proof and medical records for travel claims
  • Email communication with insurer, TPA, hospital or garage

Do not submit altered bills or inconsistent statements. Even small manipulation can convert a valid claim into a fraud investigation.

Health, motor and life insurance claims: Mistakes to avoid

Each insurance category has its own claim risks.

In health insurance, check whether the hospital is in the cashless network before planned admission. Cashless means the insurer settles approved bills directly with the hospital. If the hospital is not in the network, you may need to pay first and file reimbursement later. For planned hospitalization, seek pre-authorisation in advance wherever required.

In motor insurance, never repair the vehicle before informing the insurer if the policy requires surveyor inspection. Take photos of damage, note the accident location, and file an FIR where there is injury, theft, third-party damage or major loss. Driving without a valid licence, using a private car for commercial use, or making unauthorised modifications can create rejection risk.

In life insurance, nominees should know where policy papers are kept. Update nominee details after marriage, divorce, birth, death or any major family change. A claim may get delayed if nominee records, identity proof or relationship documents are unclear.

IRDAI claim rules and appeal options after rejection

If your insurer rejects or partially settles a claim, do not stop at a phone call. Ask for the rejection reason in writing. Compare it with the policy wording, proposal form, exclusion list and claim documents.

You can then follow this escalation route:

First, file a written complaint with the insurer’s grievance cell. Attach policy number, claim number, documents and a clear explanation. If the response is unsatisfactory, escalate through IRDAI-linked policyholder channels such as Bima Bharosa, where applicable. You can also approach the Insurance Ombudsman for eligible disputes within the prescribed time limit. For high-value claims, consult a legal expert.

IRDAI’s policyholder portal is a useful reference for claim rejection and grievance guidance. However, your exact rights depend on policy wording, facts of the case and applicable regulations.
